PASSPORT PROCEDURES
– 01 declaration according to form X01 (fully filling out columns and sections according to the instructions in the Comment section of the declaration).

– 02 recent photos, size 4 x 6 cm, face straight, bareheaded, without stained glass, white background (01 photo pasted onto the frame above the declaration, 01 photo stuck to the back of the declaration).
– When submitting the dossier, the valid ID card must be presented. If temporarily staying, they must produce more temporary books issued by competent agencies for checking and comparison
For children under 14 years of age:
– A passport application, made and signed by a parent or father; If performed by the adoptive mother, adoptive father or guardian, a copy of papers proving that the mother, foster father or legal guardian is enclosed. The declaration must be certified and stamped by the police of the commune, ward or township where the child permanently resides (or temporarily resides).
– Submit 01 copy of birth certificate (if it is a snapshot, produce the original for checking and collating).
– In case of a child under 9 years old applying for a passport sharing with his mother or father, he / she must fill in the same declaration form of the mother or father (photo size of 3 × 4 cm of children stuck to the frame for children).
